Death certificate uses, for the right to property, to claim insurance, for family pension Birth registration is a child's primary right and is the first step in establishing his or her identify. Under the Birth and Death Registration Act 18, It is mandatory to report every birth and death to the Registrar. In most of the rural area still women give birth to child at home without going to hospital so in this case it is the responsibility of head of the family to inform and register the birth and death to the Registrar.

In urban areas, most of the birth is done with the help of maternity hospital and under surveillance of doctor so in this case it is responsibility to the officer of hospital to report such information to registrar office. 

It is necessary registered in the same area where the birth or death took place. below mentioned form number is generally use for Birth-Death and Stillbirth

- Form no.1 is for registration of Birth

- Form no.2 is for registration of Death

- Form no.3 is for registration of Stillbirth 

Online application for Birth and Death certificate programmer is develop by National Informatics Center, Gujarat State for the Department of Health and Welfare, Gujarat Government.

In addition, birth or deaths occurring between 1st April 2021 to 31st June 2021 are exempted from affidavit for registration of delayed birth or death of more than 22 days but up to 365 days which cannot be registered within the stipulated 21 days.

How to Online Apply ::

1. First open official website of Gujarat government https://eolakh.gujarat.gov.in

2. Now Click on DOWNLOAD CERTIFICATE showing in home page of site 

3. Now select event Birth or Death

3. After selected event then search your certificate by two option Application number or Mobile number

4. Enter Application no. or Mobile no. as per your preference and enter year then click on search date

5. Now it shows your certificate and click on download and its done.

Require Document for online Birth Certificate ::

• Proof of birth place (which is given by hospital)

• Aadhar card of Mother or Father

• Marriage Certificate if need be

Require Document for online Death Certificate ::

• Aadhar Card of Died Person

• Applicant Aadhar card

• Date and Time of day of death

• Notes given by Cemetery (samshan)

• Medical certificate if applicable
